Ingat teknik menjawab amat penting sebab GRE menggunakan sistem Computer Adaptive Test(CAT).10 soalan pertama amat penting sebab dia menentukan markah keseluruhan. Jadi tumpukan dan jawab betul-betul 10 soalan pertama.<br /><br />2. Buat latihan simulasi banyak2. Kat dalam internet berlambak sumber latihan GRE percuma (yang ciplak pun banyak).<br /><br />3. Buat latihan dalam POWERPREP yang dibekalkan oleh ETS. Biasanya ETS akan bagi CD percuma, kalau tak ada bileh download di laman web GRE. Beberapa hari sebelum exam buat exam CAT. Hebat kerana di hadiri oleh ramai veteran ALKIS(dato presiden, abang Aziz aka abang Roi , Pak din dll) Hebat juga sebab program dapat berjalan sehingga asar walaupun di dalam hujan lebat.<br /><br />Program bermula sekitar jam 9.30 pagi dengan ucapan alu-aluan Tok Batin dan Dato presiden. Kemudian diikuti dengan ceramah ringkas Ustaz Arif (kesihatan rohani) dan Dr Zainal (kesihatan Jasmani). Seterusnya pemeriksaan kesihatan . Alhamdulillah ramai doctor perubatan warga Alkis yang hadir (Dr burhanudin Busu, Zuridah, Ramzi dll) juga dibantu para pelajar perubatan Cyberjaya Medical College. Dari data yang dikumpul 47% yang hadir adalah non muslim (animisma, Hindu dll).<br /><br />Sementara Ust Arif memberi rawatan rohani anak-anak pula dihiburkan dengan pelbagai aktiviti oleh remaja PEPIAS. Berbanding dengan anak-anak OA di pendalaman yang pemalu anak2 OA di sini agak agressif dan proaktif.<br /><br />Menjelang tengahari masing-masing gelisah kerana makanan belum sampai. Rupanya Dr Firdaus balik ke Klang mengambil makanan yang dimasak di rumah beliau. Jalan jam pulak. Jadi kita solat dulu.<br /><br />Lepas solat makanan masih belum tiba, apa nak buat? Perut dah lapar, cuaca makin gelap. Mujurlah Ustaz Arif menyelamatkan keadaan. Jom kita dengar ceramah Ustaz dulu….ceramah ustaz memang best saling tak tumpah macam bondanya Ustazah Fauziah Sani.<br /><br />Sementara tu budak –budak yang ada kutu kita buat head lice treatment….alamak towel pulak tak ada nak basuh rambut macamana…..<br /><br />mujurlah Kak ros ada , tolong belikan good morning towel di pekan Gombak.<br /><br />Habis ceramah…..masih belum nampak kereta Dr Firdaus….Tak apa, jom kita dengar Ustaz Arif bernasyid pulak…..<br /><br />….dah habis dekat 10 lagu, makanan masih belum sampai….tapi katanya dah dekat…..hujan pun makin lebat….Ustaz kata kita selawat dulu …InsyaAllah dimudahkan urusan.<br /><br />Alhamdulillah dekat pukul 3 baru Dr Firdaus sampai. Hidangan istimewa lauk udang, ayam dan daging. Kuih karipap, cucur badak dan mihun. Dalam hujan lebat makan beramai-ramai memang menyelerakan.<br /><br />Habis makan kita sambung dengan program anak-anak. Kalau ada hadiah walaupun coklat beberapa biji anak-anak memang suka. Bhg En Kasuddin Mohd Yasin, Pakarunding 5S Kaizen<br />Timbalan-timbalan Kesihatan Negeri,<br />Wakil-wakil jabatan<br />Pakar-pakar perubatan keluarga<br />Pegawai-pegawai perubatan<br />Dan semua anggota Pejabat Kesihatan dan Klinik Kesihatan dan Pergigian Putrajaya<br /><br />Pertamanya saya selaku coordinator 5S mewakili Pakar Kesihatan Awam PKPJ ingin mengalu-alukan kedatangan tuan-tuan dan puan-puan sidang hadirin sekalian ke majlis pelancaran 5S Kaizen peringkat PKPJ.<br /><br />Tuan-tuan dan puan-puan Sidang Hadirin sekalian,<br /><br />Warga PKPJ dan KKPJ amat komited didalam usaha meningkatkan kualiti perkhidmatan dan kecemerlangan system penyampaian. Sepanjang 10 tahun beroperasi klinik ini telahpun mengukir banyak kejayaan antaranya:<br />1. Memenangi Anugerah Inovasi Peringkat Negeri Selangor 2001<br />2. Menerima persijilan ISO 9001 sejak tahun 2003<br />3. Menerima anugerah perdana teknologi maklumat sektor awam 2005<br />4. Johan Pertandingan Kaunter Farmasi peringkat nasional 2007<br /><br />Namun disebalik kejayaan dan anugerah yang diterima, kami merasakan pencapaian yang ada masih perlu dipertingkatakan lagi. Justeru itu pihak PKPJ telah bersetuju untuk memperkenal dan mengadaptasi konsep 5S Kaizen sebagai salah satu inisiatif kualiti yang baru pada tahun ini.<br /><br />Dengan bimbingan pakarunding dari firma Kaizen Integrated, En Kasuddin Mohd Yasin, pihak pengurusan telah mengorak langkah memasuki tahun 2008 dengan mengadakan bengkel pengenalan konsep 5S Kaizen kepada anggota PKPJ dan KKPJ.<br />Ianya disusuli pula dengan langkah yang lebih serius iaitu melantik ahli jawatankuasa pemandu bagi mengetuai unit-unit promosi, latihan dan audit dalaman. Setiap inci dan setiap sentimeter pelan lantai pejabat dan klinik telah dibahagi-bahagikan kepada zon yang berbeza. Setiap zon pula dipecahkan kepada subzon. Dan setiap zon dan subzon ini mempunyai ketua dan hakmilik masing-masing.<br /><br />Unit Promosi yang diketuai oleh PPP En Rosman Jonet telah bertungkus lumus mempromosikan 5S Kaizen melalui acara-acara yang pelbagai. Satu pertandingan mencipta logo 5S telah diadakan bagi mencungkil bakat warga kerja, gantungan poster dan kain rentang 5S telah dibuat bagi menyedarkan warga kerja PKPJ dan KKPJ bahawa slogan 5S yang mereka anuti suatu ketika dulu iaitu: <span style="font-weight: bold;">Selit, sumbat, sorok, serabut dan selekeh</span> perlu digantikan dengan slogan 5S Kaizen: <span style="font-weight: bold;">Sisih, Susun, Sapu, Seragam dan Sentiasa amal</span>. Buletin 5S telah diterbitkan dan lawatan kerja ke beberapa pusat kecemerlangan 5S iaitu Hospital Kajang dan MBSA telah dilakukan.<br /><br />Unit Latihan pula yang diketuai oleh Pegawai Farmasi Puan Muslisah telah berusaha sedaya upaya mengumpul dan mengadakan sesi ceramah dan taklimat kepada seluruh warga PKPJ dan KKPJ supaya tidak seorang pun yang tertinggal, bermula dari pihak pengurusan dan pakar perubatan sehinggalah pencuci lantai dan pengawal keselamatan.<br /><br />Tuan-tuan dan puan-puan sidang hadirin sekalian,<br /><br />Unit yang paling penting ialah Audit dalaman yang diketuai oleh Dr Suzana Budarto. Bengkel audit telah dilakukan bagi melatih juruaudit mengenai tatacara selok belok pengauditan. Seterusnya audit percubaan dilakukan diikuti dengan audit dalaman yang sebenar yang dilakukan secara berkala. Untuk menyuburkan lagi amalan 5S ini unit audit dengan kerjasama unit promosi telah mengadakan pertandingan antara zon dimana zon yang mendapat markah tertinggi akan menerima piala pusingan sumbangan Dr Nora’i.<br /><br />Tuan Puan sidang hadirin sekalian,<br /><br />Sehingga kini tahap kepatuhan dan keakuran 5S Kaizen dikalangan warga PKPJ dan KKPJ adalah sekitar 60-70 %. Sungguhpun begitu program 5S ini telah dapat meningkatkan penghayatan budaya korporat terutamanya di dalam memupuk semangat kerjasama dan berpasukan di kalangan anggota. Ianya juga diharap dapat menjadi pelengkap kepada kemahiran soft skill yang dipupuk melalui Konsep <span style="font-weight: bold;">10S Kejururawatan iaitu: Salam, Senyum,Sopan,Segera,Segak, Sensitif, Sentuh, Selia, Selidik dan Semangat.</span><br /><br />Insya Allah dengan berkat kesungguhan dan kegigihan semua warga kerja PKPJ dan KKPJ kami berharap supaya tahap kepatuhan ini dpat mencapai 100% bagi melayakkan kami menerima persijilan 5S peringkat nasional.<br /><br />Tuan Puan sidang hadirin sekalian,<br /><br />Sebelum saya mengakhiri ucapan ini. Chao also means Good bye, so people will say Chao when they meet and they will say the same thing when they leave.<br /><br />My mission in Venice was to present on behalf of the Ministry of Health Primary Care Division, the “Teleprimary care - Casemix Project” at the 23rd Patient Classification System International Working Conference. In collaboration with the John Hopkins University (JHU) and jointly sponsored by the WHO this research project is the first Malaysian experience of analyzing the primary health care data using the casemix software. The project has been lead and managed by Dr Safurah Jaafar and the Teleprimary Care (TPC) team. It is an organisation that creates a network of researchers and users of the casemix concept from health administration, government agencies, and academia. From an initial focus on the Diagnosis Related Groups (DRG), the association’s goals have expanded to include a broader interest in clustering and grouping techniques of clinical and administrative data for health care management and financing. The association has stimulated the use and refinement of the science of "re-grouping patients" to create classes, which are relatively homogenous in respect of the resource used by taking into account severity of illnesses, patient age, co-morbidities and complication. This new group of patient database is extremely useful for risk adjustment, capitation, predictive modeling and resource distribution. Traveling a long haul flight in a Malaysian Airline Business Class cabin allow you to relax and have a good rest. The seat is spacious, comfortable and can be transformed into a flat bed. It is also equipped with a ten minutes massager and a multi-channel in-flight entertainment. Even the food has so much varieties and course; one after another. The in-flight entertainment even has an interactive online fact of the popular travel destination all over the world together with a language lesson. I managed to learn a few basic Italian languages while on board.<br /><br />It was 7am Malaysia time, almost 6h on board when I was awakened by the air steward. A view outside the window was still dark and the navigator on the small monitor in front of my seat showed that the plane was flying somewhere above a desert of Afghanistan. The air steward asked me whether I am a medical doctor and whether I could see a French boy who suffered from diarrhea and vomiting. I showed him my MMA card and agreed to see the boy.<br /><br />‘Bonjour’ I greeted the boy and his parents in French. After introducing myself I took a brief history from them followed by a quick physical examination on the boy. It seemed that the boy had acute gastroenteritis and oral medication given earlier by the air steward seemed to be ineffective. I asked the steward whether they have any parenteral medication. He then led me to a medical container that only permitted to be opened by a certified medical professional. Fortunately I found a few ampoules of Stemetil and Oral Rehydration Salt inside it. After discussing with the boy and the parents they agreed for the injection. The chilling temperature outside was 5 degree Celsius. The airport buildings are scattered in one large area that you have to take shuttle bus and train to its various terminals. The congested tunnel shaped terminal seemed almost claustrophobic. I have to join half a mile queue to get my passport stamped. The immigration officer welcomed me with a grim, stern- faced and only let me go after I showed him the conference registration letter. Soon after, everyone had to go through a stringent safety screening before we were allowed to enter the departure gate.<br /><br />I left for Venice with Air France and it took me two hours to reach the Marco Polo International Airport. The Aeroporto di Venezia Marco Polo as the Italian called it, is named in honor of its famous citizen. It is situated around 7 km north of Venice, on the edge of the lagoon. From there you have few options to enter Venice either by land or by sea. World-famous for its canals, Venice was built on an archipelago of 118 islands formed by about 150 canals in a shallow lagoon. Its history can be read in every monument and pallazo, in fact each and every stone bears witness to the period of splendor, decay and wars. The city could be described as a gigantic open air museum, visited each year by million of tourist from all over the world. Founded fifteen hundred years ago, Venice rose to become Europe’s main trading post between the West and the East, and at its height controlled an empire that spread north to the Dolomites and over the sea as far as Cyprus. The <em>Brasilica di San Marco</em> is the most mystical and exotic of Europe’s cathedral. Dan Brown, of <em>Da Vinci Code</em> fame, would have had a field day here. The combination of ancient structure and later decorations is to a great extent what makes the cathedral so bewildering. In contrast, the adjacent building of <em>Pallazo Ducale</em> is decorated with a geometric patterned stonework resembles an Islamic art. Perhaps this is one of the Islamic architecture that remains intact. Venice's economic and diplomatic relationship with the Islamic world from seven century onward is well documented. As Byzantium gradually gave way to Islamic caliphates from the eighth century onward, meeting its ultimate demise in 1453 at the hands of the <a href="http://www.metmuseum.org/toah/hd/otto1/hd_otto1.htm">Ottoman</a> Sultan Mehmet II, Venetians increasingly came into contact with Muslims and their ideas, culture, and way of life. As a result, Venice became Christian Europe's most important interface with the Muslim civilizations of the Near East. Many of the top-flight Italian designers and fashion houses are here; Gucci, Armani, MaxMara, Prada, Valentino, Benetton, Stefanel etc. For those with wallets as deep as oil wells this place is the most fruitful zone. If you are like me who always buy things at ‘pasar malam’ and during cheap sale carnival this place is only good for a window shopping.<br /><br />As one of the most exorbitant city in Eastern Europe you would expect everything is expensive in Venice. A small hotel that I stayed cost me 140 Euros (RM670) per night. An hour of gondola ride through the canals will cost you 160 Euros (RM 768). The exact venue was at the Palazzo del Cinema where the annual Venice Film Festival (the world’s oldest and the most important film festival in Europe after Cannes) is usually held. In contrast with the bustling Venice, Lido is amazingly quiet, serene and peaceful to the point of pleasant somnolence.<br /><br />With more than 300 participants from 36 countries all over the world, the conference was a great sharing forum. Every speaker presented their own project and enlightened their experience of using and implementing casemix system in their country. Safurah was right when she persuaded me to pursue into this area. Not everybody who involved in the casemix project has a degree in Public Health, there are pediatrician, psychiatrist, rehabilitation physician, nurses and even economist. To my surprise the person who presented the new version of German DRG is a surgeon!!!<br /><br />As the last speaker that day my nervous eased a lot after seeing how the others performed. It was always my dream to go deep into the heart of <st1:place>Borneo</st1:place>, the place where even most of the Sarawakian never been.<span style="color:red;"> </span><br /><br />The only way to Kapit and Belaga is by express boat through the liquid highway of the mighty Rajang river. The elongated twin horse engines express boat measured approximately 15 meters long offered 3 different categories of sit ; the first, second and third class. A fully air-conditioned cabin with a large screen TV at the front provides a comfortable and relaxing journey. There are several trips going upstream, the last trip is at <st1:time minute="30" hour="12">12.30pm.</st1:time><br /><br />We started our journey from the Sibu wharf by taking the afternoon express boat towards Kapit. The plan was to spend a night in Kapit and proceed to Belaga the next day. In contrast with the traditional wooden or bamboo made long house the modern one is almost similar to the terrace houses in town.Occasionally we encountered the tugboats hauling massive log rafts from the interior to the downstream timber mill. The murky yellow ‘teh-tarik’ like, muddy water of the mighty Rajang reflects the heavy logging activity in the upstream.<br /><br />At the distance I caught sight of the log-loading station at one side of the riverbank. <span style="font-size:0;"></span>I couldn’t help but felt upset to see the bulldozers and huge lumber trucks moved back and forth over the large clearing, arranging the giant logs. As the defeaning roar of the engine ceased away it nestled in adjacent and parallel with half a dozen similar boats. We have to walk cautiously on the edge of the adjacent boat before able to step on the concrete building of the wharf.<br /><p class="MsoNormal">Kapit is the homeland of Iban. It is the largest division in <st1:place>Sarawak</st1:place> . It consists of three districts; Kapit, Song and Belaga. Kapit town itself<span style="font-size:0;"> </span>is the division’s administrative centre. Most of the lands are covered by dense primary forests. The mighty Rejang River and it tributaries; Batang Baleh , Batang Katibas and Batang Balui cut through the division and remain the only means of transportation for the people traveling into the heart of Sarawak. Due to the mountainous topography and under populated area the Malaysian Airlines had stopped the aircraft service to Kapit long time ago.<br /></p>There are two versions of story telling how Kapit got its name. The first version claimed that the word ‘Kapit’ was the evolution of ‘Kepit’ which means the bamboo longhouse. When people were going to the longhouse they would said; “kami ngagai rumah panjai kepit” which means we are going to kepit longhouse. The Orang Ulu refers to a number of smaller ethnic group that settle upriver mainly from Nanga Merit area up to Belaga. They are further divided into small subethnic; Kayan, Kenyah, Punan, Sekapan, Kejaman, Lahanan and Tanjong. (Please note that Punan and Penan are two different ethnics.)<br /><br />That evening I have a separate agenda at the Kapit Health Clinic while Dato Shafei and the rest of the team went straight to the Sri Balleh government rest house on the top of the hill overlooking the Kapit town where we spent the night. </p>After dinner we have a chit-chat. We were talking about the famous yet the most dangerous Pelagus Rapid that we were going to pass through on our way to Belaga the next morning. Traveling upstream depends very much on the rain and water level. If there is too much rain, the logs and branch debris make river travel unsafe. Too little rain and the low water level exposing<span style="font-size:0;"> </span>long stretches of dangerous rapids. Even when the river conditions were ideal, accidents could happen as a result of a moment’s inattention. The former Kapit Divisional Health Officer, Dr Rais Abdullah (Krishnan) was telling us that he only trust one boat driver from Belaga if he was to make a trip there. That is because not everybody has enough skill to cruise through the rapids. The boat driver must have a precise knowledge of every single submerged rock of the rapid. Any single mistake may result in catastrophe.<br /><p class="MsoNormal">Almost every year the rapid claims human soul. Early this year a school teacher from the interior school died after been accidentally thrown away by the wobbly boat into the rapids while he was enjoying the view on the roof of the express boat.<br /><br />That morning after the dawn prayer I took a walk around the rest house. The extensive spectacular panorama of the greeny mountains and the slow moving river filled my heart with melancholy. I realized that I have strong feeling towards the river, the mountains, the forest and the surrounding people. </p>While waiting for the express boat to Belaga I paid a visit to the Sylvia Port. Unfortunately (or rather fortunate) the inner cabin was full with passengers and only few of them alighted to end their journey in Kapit. The overwhelmed excitement overcomes my sense of fear as the express boat speeding upstream. I anticipated the ‘Lost World’ of Arthur Conan Doyle and forgot the dangerous man-swallowing rapids.<span style="font-size:0;"> </span><?xml:namespace prefix = o /><o:p></o:p><br /><br />Sitting on the roof was actually a perfect place to enjoy the beautiful panorama of the rainforest. The blue sky on the top combined with the murky yellow river at the bottom and the greeny mountains on both sides gave you an almost surreal landscape. As we were approaching the Pelagus rapids I could see a warning signboard on the riverside alerting the boat passengers to put on their safety jacket. Amazingly I could not find a single safety jacket available for the passengers inside the boat let alone for the extra passengers on the roof-top.<br /><br />The slow moving water suddenly became a terrifying<span style="COLOR: rgb(255,102,0)"> </span>tumultuous turbulence. As the express boat lurched and rolled it way through the boiling rapids I strengthened my grab on the handle while my mouth keeps repeating the Noah prayer. I could felt my adrenaline flows freely as the river and my entire system was fully charged for fight or flight should any bad things happened. But thanks God we managed to cruise through the treacherous rapids safely. The initial terrifying feeling later became an exhilarating experience.<br /><br /><br /><a href="http://video.google.com/videoplay?docid=-2906510774720103765&pr=goog-sl" target="_top"><span style="COLOR: rgb(102,102,102)">WATCH PELAGUS RAPIDS IN ACTION</span></a><br /><br /><p class="MsoNormal">We reached Belaga six hours later. The Belaga folks welcomed us with the traditional Kenyah dance. Belaga is such a small town. It has only few rows of shop houses, budget inns, a health clinic and a post office in one small area. Later on we went to see the site for the new health clinic where the construction is yet to start early next year.<br /><br />During dinner there was a big welcome party. Dressed in their traditional costume a group of<span style="font-size:0;"> </span>Kayan ladies performed their traditional dance followed by a solo dance of ‘ngajat’ in synchronize with the background traditional music. Later on, in his speech Dato Shafei thanked the Belaga people for their warm hospitality and reassured them that the new clinic will becomes a reality soon. The party continued with a karaoke session (a compulsory session for the Sarawakian during any party) and it end up an hour before <st1:time minute="0" hour="0">midnight</st1:time> with a traditional dance of ‘Li-ling’ where everybody walked rhythmically in a circle.<o:p></o:p><br /><br />At dawn I was awakened by the chorus of azan from the nearby mosque. After having a light breakfast we caught an early express boat back to Sibu. A brief visit to this remote area of <st1:place>Sarawak</st1:place> was indeed a rewarding experience. The fact that there were only three of us who were the participants as well as the speakers was indeed interesting. Sibu has been given an honor to become a host. The guest speaker that day was Dr. William Chung, a nephrologists from Normah Medical Centre. He is a Chinese American based in the Mayo clinic and currently working as a contract nephrologist in the private medical centre. Since there was only a small group of us at the meeting it has became an informal discussion and everybody was in relaxing mood. After been occupied with series of lectures and discussion since afternoon, we have our dinner in one of the Melanau restoran. The Melanau cuisine offered a variety of sea food ranging from the famous umai, sago snack, asam pedas and fried ikan kurau in sweat and sour sauce. Later that evening we went for a night tour along the riverside. Everyone was taken aback to observe the tremendous changes Sibu has gone through…from literally a plain, dull ‘cowboy’ town into a modern and contemporary municipality. We were spellbound by the spectacular view of<span style=""> </span>Wisma Sanyan; the tallest building in Sarawak standing bold as the epitome of this hardworking <st1:place st="on">Foochow</st1:place> community. Setelah lapan tahun berehat kami diberi pilihan sama ada menjalani kelahiran normal atau pembedahan. Memandangkan faktor usia dan jangkaan bayi yang besar, kami akhirnya memilih kelahiran secara pembedahan. Bayi kali ini memang dijangka besar walaupun tidaklah seberat kakaknya Maisarah (4.45kg).<br /><br />Sudah menjadi adat tinggal di perantauan jauh dari mak ayah dan saudara mara, sayalah terpaksa mengambil alih tugas memasak, membasuh dan mengemas rumah. Mujurlah ada rakan-rakan dan kakitangan klinik yang sudi menjenguk. Semoga menjadi zuriat yang solehah , alimatan dan amilatan. Ia nya telahpun dirasmikan oleh Ketua Menteri Sarawak, YAB Datuk Patinggi Tan Sri (Dr) Haji Abdul Taib Mahmud Pada 24 April yang lalu. Dengan siapnya jambatan ini nanti penduduk yang akan ke Sarekei dan seterusnya ke Selatan termasuk Kuching tidak perlu lagi beratur menaiki feri. Beratur menunggu feri kadang-kadang sampai 3-4 jam terutama sewaktu musim perayaan.Walaubagaimanapun selepas ini mungkin terpaksa beratur untuk membayar tol pula. Jambatan ini merupakan laluan bertol kedua di Sarawak setelah jambatan Petrajaya di Kuching.
